Your foot or ankle concern, medical history, current medications, allergies, and past surgeries. This helps us prepare for your visit.
Consent and Privacy
Consent for treatment, HIPAA acknowledgment, and an optional disclosure form if you’d like us to discuss your care with a family member.

No insurance? No problem. We’re happy to discuss payment options when you arrive.
What to Expect at Your First Visit
Plan to arrive about 10 minutes early so our front desk can review your paperwork and get you checked in. Your appointment will typically include a thorough examination of your feet and ankles, a discussion of your symptoms and goals, and a recommended treatment plan. Most first visits last about 30 to 45 minutes.
